It's all about fine control of every aspect of getting the image, control of the light and shadow including both amount and nature, the pose, exposure and so forth. With that in mind, the camera might know how much light is there, but not its quality, so I shoot exclusively with manual settings. I sometimes use auto focus, but only because of 'old' eyes. I don't recommend it!
Outdoors, the best times for shooting people would generally be a couple of hours after sunrise or before sunset. But, with scrims and other light control devices, it makes little difference. I greatly prefer reflectors over fill flash, but diffused fill flash is often acceptabel and necessary. I don't recommend undiffused fil flash at all, it's far too spectral.
