I suppose this is intended as some kind of Scary Story. Let me make it more real.

About 30% of my business is with the pharmaceutical industry, and we have to deal frequently with exactly that question. When a job for herpes, HIV or any other stigmatized disease drug comes through we always have to ask our models if they would be willing to do it. We have found that about 85% of them say yes. Most people are able to distinguish between a role in an ad and real life.

That particular commercial was a SAG National, and as always pays at least SAG scale. You don't get to negotiate something else - it is what it is. Typical pay for a SAG National commercial that plays nationwide for an extended period (as this one is likely to do) is about $30,000-$50,000 for principles, depending on how long it is used. The vast majority of models will work on Herpes commercials for that kind of money.
: 3. Given the TFP release... what makes you think you aren't that face in... Hong Kong?
Simple answer: virtually nobody who does TFP on the net has any ability to sell pictures to someone doing pharmaceutical commercials in Hong Kong. So the chances are very, very remote.
But in real-world terms, a TFP is equivalent to shooting Stock Photography. The difference is that in a TFP you get prints which, if you have chosen your photographer well, will advance your professional career. In Stock, you get a small amount of money (typically $100-$500). The release is the same, and the chance that a picture submitted to a Stock Photography house would get used by a pharmaceutical company is much greater than the chance that a TFP picture from a web photographer would.
Many agencies do not support stock work; about as many do. At those that do, roughly 70% of models will take stock assignments; older, more established commercial models are less likely to.
